If you are planning a trip abroad and require any vaccinations:
you should complete and submit a travel questionnaire at least two calendar months prior to your scheduled departure date.
We are unable to process and therefore cannot accept late requests to use this service.
Any patient travelling within two months should contact the MASTA travel clinic.
The nearest private travel clinic is:
Masta Travel Clinic
Axminster Medical Practice
St Thomas Court
Telephone: 033 0100 4 107
Website: MASTA travel clinic
Once we have received your completed questionnaire, the practice nurse will assess your needs and will advise you what vaccinations are required and what medications you may need. We will need to contact you to give this advice and to make the necessary follow-up arrangements (schedule vaccinations/book appointments) once everything has been signed off by your doctor.
Please ensure that your contact details are up-to-date and that you supply a day time/mobile telephone number.
These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least two weeks before you travel to allow the vaccines to work. The practice offers all major vaccinations currently recommended, including yellow fever. Bridport Medical Centre is a registered yellow fever centre. Please note however, that there is currently a national shortage of yellow fever vaccine. Visit NaTHNaC to find a designated yellow fever vaccination centre.
Please note that advice for malaria changes from time to time, so please visit this website for the latest advice: Fit for Travel.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
